Please remember to include the following:

  • Copy of W-9
  • Copy of state contractor’s license
  • List of company services and manufacturers represented
  • List of project experience (current projects/completed projects)
  • References
  • OSHA 300 logs from the last 3 years
  • Independent verification letter supporting your EMR for the last 3 years
  • Certificate of insurance (in accordance with Exhibit D of the Cannon Master Subcontract Agreement)
  • Financial statement
